Fallout 76 Mutations Guide: All 19 Mutations Ranked
Fallout 76 mutations are semi-permanent buffs with a downside that you get from radiation or serums, and with the right perk cards the downsides almost vanish. Many beginners panic and remove them with RadAway without knowing that Marsupial and Speed Demon are among the strongest buffs in the game. This guide lists all 19 mutations with a rating, explains how to get and keep them, and shows a mutation set for every build.
Understanding Fallout 76 mutations: the basics
Each mutation has a positive buff and a negative debuff. You can have up to 18 mutations at once, because Carnivore and Herbivore block each other. With the right perk cards, the drawbacks become barely noticeable.
How do you get mutations? Two ways: randomly through radiation (5% chance per 5 points of radiation damage, from level 5) or on purpose through mutation serums. Serums are the better method, because you pick exactly the mutation you want.
Where do you get serums? From player vending machines, which are usually the cheapest option, or from the MODUS Science Terminal in the Science Wing of the Whitespring Bunker, which sells both serums and serum recipes. A serum grants its mutation and suppresses the negative effect for one hour.
The three must-have perks for mutations
Before you take a single mutation, you need these perk cards:
Starched Genes (Endurance, one rank, level 30): You never mutate from rads, and RadAway never removes your existing mutations. Without this perk, every dose of RadAway can remove a mutation.
Class Freak (Luck, rank 3): Cuts the negative effects of all mutations by 75%. That turns “-4 Strength” into “-1 Strength”, a barely noticeable drawback. Available from level 46.
Strange in Numbers (Charisma, rank 1, level 42): Boosts the positive effects of your mutations by 25% when you are on a team with other mutated players. Since almost everyone has mutations, the bonus is active nearly all the time.
Careful with Rad-X: According to fallout.wiki, the full version suppresses the positive effects of your mutations for 10 minutes, even with Starched Genes. Use diluted Rad-X or RadShield instead for radiation protection without side effects.

Farming mutations: step by step
There are two ways: on purpose with serums (recommended) or randomly through radiation (cheap but slow).
Method 1: Buy serums (recommended)
Step 1: Equip Starched Genes and Class Freak (rank 3).
Step 2: Visit player vending machines on the map. Many sell mutation serums, and it is usually the cheapest method. Our caps farm guide helps you fund them.
Step 3: Take the serums in the order you want. After taking one, its negative effects are suppressed for 1 hour.
Step 4: Done. As long as Starched Genes is equipped, your mutations stay permanently.
Method 2: Radiation (cheap, but random)
Step 1: Temporarily remove Starched Genes from your perks.
Step 2: Expose yourself to a radiation source (nuclear barrels, irradiated water, nuke zones). Every 5 points of radiation damage gives you a 5% chance at a random mutation. Once you get a mutation this way, you cannot get another from radiation until you leave the server.
Step 3: If you get an unwanted mutation, take RadAway (without Starched Genes) to remove a random mutation. Repeat until you have the ones you want.
Step 4: Once you are happy, equip Starched Genes again.
Pro tip: Combine both methods. Farm three or four mutations through radiation, then buy the rest as serums. That saves caps.
Common mutation mistakes
Taking full Rad-X: Suppresses the positive effects of your mutations for 10 minutes. Diluted Rad-X or RadShield are the safe choice.
RadAway without Starched Genes: Every dose of RadAway has a chance to remove a random mutation. Always check that Starched Genes is active first.
Using a decontamination shower: According to fallout.wiki, decontamination showers also have a chance to remove a mutation, just like RadAway. Some community reports claim this can happen in certain quest situations even with Starched Genes, but that is not confirmed. Be careful during “An Ounce of Prevention”!
Carnivore and Herbivore together: Not possible. You can only have one of them. Pick based on your preferred buff recipes.
